Complete the application form in English, using the Passport Wizard. Print pages 5 & 6 and bring with you on the day of your appointment.
Item 5 – Social Security Number: You must complete this item in accordance with 22 USC 2714a. If you were never issued a social security number, you/your parent/legal guardian (for minors) must submit a declaration under penalty of perjury stating that you were never issued a social security number. Failure to provide a Social Security Number will result in suspension of your passport application. If you have a number, but do not remember it, call the Embassy’s Federal Benefits Unit for assistance. You may contact the Federal Benefits Unit Monday, Tuesday, Thursday and Friday, 9:30 a.m. to 11:30 a.m. at 210-720-2426 or by e-mail at FBU.Athens@SSA.gov
Item 6 – Complete this item so that the Embassy can notify you when your passport is ready.
Item 8 – Mailing Address: Complete this item with a local address, either your home address or the address where you are staying. This information will be used for the delivery of your passport.
Item 17 (Item 18 on DS-82) – Permanent Address or Residence: Complete this item with any address you consider to be your permanent address. This need not be in Greece or the U.S.
Items 18 &19 (Items 19 & 20 on DS-82) – Travel Plans and Emergency Contact: This information is not required, but may be very helpful in processing your application and in assisting you in the future. If your emergency address is not in the U.S. enter “AE” in the box requesting a state – this will allow you to continue completing the application. We will erase it when processing the application.
Oath & Signature (DS-11) – You are required to sign in the presence of the consular officer at the Embassy. You must read the “Acts or Conditions” as outlined in the instruction pages before you sign. If any mentioned acts or conditions have been performed by or apply to the applicant, cross out only the applicable portion(s) and submit an explanatory signed statement with the application.
