• Your current primary full validity passportand/or your previous second passport (if applicable);
• Two U.S. passport sized (2x2inch) photographs– see photo requirements  Photos which do not meet the requirements will be rejected; Greek passport size photographs are not accepted;
• A statement from your company in support of an application for a second passport.
